Q Download the Core Measure Criteria for stroke patients from the Joint Commission’s website hereLinks to an external site. (scroll down to the Stroke STK section). Using the data collection form provided record the patient information from the stroke cases. There are two patient records provided here for you: Case 1 is on John Doe; Case 2 is on Catherine Brown; Next, you should abstract the stroke criteria from the patient records. Complete the Excel file called Stroke Data Collection Form (open it and then File>Make a Copy to input your data). Basically what you need to do for each patient is review the chart against each of the STK criteria, then say yes or no if that criteria is met. If it IS MET, you need to indicate where in the chart you located it -i.e. discharge summary. Finally, develop a report of your findings. PreviousNext
